use crust, or wet it first and then squish it round the hook, or rap the bread in bait elastic round the hook

U know as stupid as it sounds it also depends on the quality of the bread,bought some cheap stuff from Netto and it refused point blank to stay on the hook

use a nice fresh uncut loaf,the crust seems to stay on longer,
alternately try some of the artificial baits on the market,you can buy plastic dog biscuits and tiger nuts,they both float and take fish,my favourite is plastic bread flake,it looks like the real thing and it floats
