Best knot?

I've always used a half blood knot for attaching lines to hooks & swivels etc. This weekend I lost one fish and nearly a second, just managed to get it in the landing net (28lb mirror)
I've never had any problems before and I always leave a reasonable tag end so why has this happened now ? I'm using a fluorocarbon leader.

Can anyone suggest a better, more reliable knot?

Floro is a nightmare try the following Knot for the floro, this is what I use.
